Biological Activity:
Galunisertib (LY-2157299) is a selective, orally active, small-molecule TGF-beta receptor type-1 inhibitor in phase II clinical trials for the treatment of hepatocellular carcinoma, recurrent glioblastoma and unresectable metastatic pancreatic cancer, and in phase II/III trials for the treatment of myelodysplastic syndromes. In in vitro studies, galunisertib inhibited TGF-beta-mediated Smad phosphorylation in human umbilical vein endothelial, glioblastoma, hepatocarcinoma and hematopoietic myelodysplastic cell lines. In triple-negative breast cancer cell lines and in human breast cancer SUM159 xenografts in mice, galunisertib blocked cancer stem-like cell expansion. In a phase II clinical trial in 106 patients with advanced hepatocarcinoma, galunisertib showed a manageable toxicity profile and an overall time to progression of 12.0 weeks. Studies are ongoing with galunisertib alone or combined with sorafenib.
