Mild 250mg q6h (toal daily dose = 1.0) 500mg 6qh (total daily dose = 2.0g) Moderate 500mg q8h (total daily dose = 1.5g) or 500mg q6h (total daily dose = 2.0g) 500mg 6qh (total daily dose = 2.0g) or 1g q8h (total daily dose = 3.0g) Severe, Life threatening only 500mg q6h (total daily dose = 2.0g) 1g q8h (total daily dose = 3.0g) or 1q6h (total daily dose = 4.0g) Uncomplicated urinary tract infections 250mg 6qh (total daily dose = 1.0g) 250 mg 6qh (total daily dose = 1.0g) Complicated urinary tract infections 500mg 6qh (total daily dose = 2.0g) 500 mg 6qh (total daily dose = 2.0g) Paediatric DoseChildren > 40kg: Normal adult dose.Infants & Children < 40 kg: 15mg/ kg at six hours intervals.The total daily dosage not to exceed 2g/ day.
